Surgical treatment of hypophyseal adenomas, complications

Abstract

In endocrinologically active hypophyseal adenomas is surgical treatment first method of choice, besides prolactinomas. These have specific indications for surgery in non-respondents and in some cases of visual deterioration.

Transsphenoidal approach is preferentially used, mostly endonasally, sometimes endoscopically. Normalization of endocrine functions fluctuates in m.Cushing between 67-86% and in STH adenomas between 42-84%.

Inactive adenomas are usually bigger and present by hypopituitarism or visual symptoms. Radicality can be increased by use of CO2 laser and neuronavigation.

Subfrontal approach is exceptionally used for parasellar and complex lesions.
