Changes of the osteoblastic activity on 18F-NaF-PET/CT in patients with treated prostatic carcinoma

Abstract

Aim. To evaluate the possibilities in the assessment of the consequences of antiandrogen therapy in men with treated prostatic carcinoma during the evaluation of 18F-NaF-PET/CT.

Method. In 30 consecutive examinations of 18F-NaF-PET/CT in men with treated prostatic carcinoma were assessed the changes in the osteoblastic activity like high-turnover osteopenia, and especially the changes related to the insufficiency fractures.

All examinations were conducted after intravenous application of 18F-NaF in the dose of 2 MBq/kg during the steady-state period of radiopharmaceutical distribution, all CT scans were performed with low-dose protocol, without application of kosine kontrast materiál a nd with submillimeter spatial resolution. Results.

In 3 patients, the signs of high-turnover osteopenia were found, the all of patients were on relative short-term anti-androgen therapy. In 3 patients, the insufficiency fractures were found, twice in pelvic girdle, once rib fractures.

In 8 men, the increased osteoblastic activity was found within weight bearing zones; the degenerative changes with active bone turn over in 4 men respectively. Conclusion. 18F -NaFPET/CT is the important diagnostic tool which is able to detect the changes of the osteoblastic activity inn patients under antiandrogen therapy and also it is able to detect insufficiency pelvic fractures caused by accelerated osteoporosis.
