Abstract

The author reviews an up-to date opinion on the atopic dermatitis- ethiopathogenesis, genetic factors, skin barrier dysfunction, immunologic imbalance towards Th2 answer, interacton of specific and non- specific immunity factors, role of staph aureus, two-phase model of inflammatory reaction, evolution of atopic dermatitis from intrinsic through extrinsic to autoimmune phase.. She mentions the stepwise therapeutic scheme from emollients, corticosteroids, topical immunemodulators towards phototherapy and systemic treatment.

She focuses on patient education for better compliance and prevention of corticophobia; on importance of suitable compoundend forms, eczema dressing and special hygienic skin care. In systemic treatment she points out the ciclosporin A as the first choice, and emphasizes the use of corticosteroids only as rescue therapy.

Finally she appeals for psychosomatic approach to the patients.
