Patients with Barrett´s esophagus (BE) have an increased risk of esophageal adenocarcinoma. The aim of the surveillance program is early detection of Barretts esophagus related neoplasia (BORN).

Enhancement endoscopy such as narow band imaging (NBI), high resolution endoscopy (HRE) and autofluorescence (AFI) are useful in the early detection of high grade intraepithelial neoplasia (HGIN) or early adenocarcinoma (EAC). Impact of this technique on the detection of low grade intraepitelial neoplasia (LGIN) remains unclear AIMS: To evaluate the diagnostic accuracy of AFI in the detection of LGIN, HGIN and EAC in consecutive patients with BE referred to our center with suspected BORN.

SETTING: University hospital. A prospective cohort study CONCLUSION: The benefit of AFI in diagnosis of LGIN is controversial especially due to a high rate of false positivity.

However, our results suggest a significant role of learning curve. The results further suggest the over-diagnosed of LGIN in patients with BE in primary care.
