Abstract

Unstable osteoporotic intertrochanteric fractures with the defect of posteromedial cortex represent a therapeutic problem. In stable intertrochanteric fractures DHS osteosynthesis is the treatment of choice.

Unstable types are affected significantly by varous type of displacement. Comminution in the posteromedial part of the intertrochanteric region results in the collapse of femoral neck, limb shortening and risk of non-union.

Proper reduction with osteosynthesis by PFN witch stops femoral neck from collapsing represents a correct method of prevention.
