Introduction: Colorectal cancer (CR-CA) screening in the Czech Republic has been done for thirty years using guajac Haemoccult test. Quantitative immunochemical fecal occult blood test (qi-FOBT) appears to be far the best in the world at present.
This study evaluates this qi-IFOBT for screening in the Czech Republic. Patients and methods: Consecutive patients (n = 685) indicated to colonoscopy were enrolled in this study.
Immunochemical quantitative tests were run using OC- SENSORm analyser (EIKEN, Japan). Values of qi-FOBT (ng/ml) were calculated as mean of two samples, and optimal cut-off value was evaluated in the range 75 - 150 ng/ml.
Results: Values of qi-FOBT (mean ± SD) were 56.9 ± 84.2 in the control group, 47.2 ± 63.8 for small polyps 10mm and 806.8 ± 421.9 for CR-CA cases. With cut-off value 100 ng/ml we have sensitivity 80.0%, specificity 87.5%, accuracy 86.7%, positive predictive value 52.7% and negative
