There are still not known antihypertensive agents, which led to reliably reduce blood pressure less than 140/90 mm Hg in all patients with arterial hypertension. This article summarizes some proven diagnostic and therapeutic procedures used in the care of patients with resistant hypertension.
Promising therapeutic approach seems to be adding mineralocorticoid receptor blocker spironolactone to existing treatment. In the first place it is necessary to exclude low patient compliance to treatment and etiology of secondary hypertension.
After the failure of any drug, new pharmacological approaches, such as electrical stimulation of the baroreflex and sympathetic renal denervation can be considered.
