Present and future of Crohn's disease treatment

Abstract

Despite unmistakable progress in therapy of Crohn's disease, there still remains many problems and unexplained procedures in treating this disease. In a review article, the author reflects on some of them.

For the treatment of perianal Crohn's disease, limits of immunomodulator therapy and biological therapy combined, approaches to patients after bowel resection and criteria on which to base the selection of appropriate prophylactic therapy. At the end of list is this situation around mucosal healing, which is indeed very important for determining next prognosis in Crohn´s disease patient, however, monitoring of mucosal healing is far more complex than that of patients with ulcerative colitis.

For these reasons, the assessment of mucosal restitution in Crohn's disease patients has not yet become standard for routine clinical practice.
