Initial Calculated Antimicrobial Therapy in Patients with Hartmann's Procedure for Perforated Diverticular Disease of Large Bowel

Abstract

On the bases of retrospective study 38 patients with emergency Hartmann's procedure for perforated diverticular disease of large bowel were evaluted efect of antibiotics regimes. Antibiotics therapy was started after source infection control / monotherapy - piperacillin/tazobactam, or combination therapy-clindamycin or metronidazole and aminoglycoside /.Bacteriological examination of the specimen from peritoneal cavity and sensitivity of antibiotics were, determined.

Bacteriological examination isolated besides Enterobacteriacea anaerobes / 27 % / and enterococci / 19 % /. Postoperative complication were 20 % in piperacillin/tazobactam group and 22 % in group with combination of antibiotics.Overall mortality rate was 11 %.

Initial calculated therapy with piperacillin/tazobactam after operation for perforated diverticular disease was effective againts all isolated bacteria with excellent antimicrobial activity and low toxicity.
