Abstract
Problems of discharge of elderly patient form hospitals are summarised. The main task of health care institutions is to ensure an adequate sequence of hospital and ambulatory care.
It is necessary to ensure continuation of rehabilitation of the patient after his discharge and to ensure him as much self-sufficiency as possible. Team work is needed for this purpose.
Discharge of the patient must begin in sufficient time before the planned date of discharge.
