Discharging elderly patients from hospital: A professional as well as an organizational problem

Abstract

Results of the study of the process of discharging elderly patients from hospitals are summarised. In the first step the instrument for detection of risk patients was elaborated and validated.

In the second step the procedure of discharging 200 elderly patients and its results were studied. The present way of discharging patients based on care for social risks works quite well.

To improve the process it would be necessary to introduce individualised care in the form of case management. At the same time it would be important to integrate ergotherapy in the process in a more systematic way.

The importance of use of ergotherapy in the care for elderly patients is often underrated in Czech hospitals.
