Abstract
There is no national reporting system of errors or incidents in health care in the Czech Republic. Individual facilities tend to have some individual systems more or less comparable.
The authors have used a questionnaire to map the present situation in Czech hospitals and they compared individual definitions of health care incidents. They present an overview of reported incidents.
