The study is dealing with circumstances of origins of avascular necrosis of femoral head by petrochanteric fractures treated with osteosynthesis. The study refers to eight cases of avascular necrosis from the count of 1373 patients operated because of such diagnosis.

The study does not discover risc factors of the origin of such rare complications, neither mistakes in the operational technique which could cause such complications. As the most likely source which undermines the origin of necrosis is identified casualty affectin of deep branch of a. circumflexa fem. med.

The best prevention of the origin of avascular necrosis is early reduction, stable osteosynthesis and correct operational technique
