The issue of paraaortic lymphonodes in endometrial cancer

Abstract

Endometrial cancer (uterine cancer) is the most common gynaecological malignancy. Surgery, often combined with radiotherapy, is the principal treatment modality.

Postoperative radiotherapy is indicated in the presence of risk factors. Radiotherapy of regional lymphonodes is mostly limited to the small pelvis and not include paraaortic lymphonodes.

We did not record any local recurrence in the group of 36 patients (pT1c without paraaortic lymfadenectomy) after small pelvis radiotherapy. However, we recorded 4 patients with metastases in paraaortic lymphonodes and 1 patient with distant metastases.

The objective is to design a trial in which patients who underwent pelvic lymphadenectomy without paraaortic lymphadenectomy - pT1c, pT2a,b - invasion to myometrium pT1c, pN0 - will undergo postoperative radiotherapy of paraaortic lymphonodes and additional vaginal brachytherapy.
