Treatment of type 2 diabetes mellitus and metabolic syndrome during pregnancy

Abstract

If pregnancy is postponed to higher age, we could expect more frequent incidence of the diabetes complications, diabetes mellitus type 2 and metabolic syndrome even in pregnant women. The situation is associated with the exposure to drugs that should be avoided during pregnancy.

The article gives information about risk of the drug exposure to fetus development. Except for the information summary based on the newest published studies, it gives also instruction, how exposure to drugs should be managed according to recommendation of European Network of Teratology Information Services (ENTIS).

The drug of choice for treatment represents always insulin. If woman therapy has not been changed already before pregnancy, it must be done immediatelly, if woman elicits gravidity.

The same options should be done for antihypertensive therapy. Drugs for hyperlipidemia treatment should be avoided during all pregnancy.

They can be used only in life threatening situation. However, cessation of the statin treatment during the first trimester is appropriate.

The accidental exposure to drugs is not reason for elective termination of pregnancy, however intensive prenatal care and second level ultrasound examination is recommended.
