Pitfalls of the treatment with basal insulin - practical limitations of the adjustment of insulin dose based on fasting glycemia

Abstract
In the treatment of type 2 diabetes mellitus with basal insulin we encounter several clinical situations. We assess the efficacy of the treatment based on fasting glycemia, glycated hemoglobin, and the presence of adverse reactions of the treatment, such as hypoglycemia and weight gain.
Based on these factors we assess and/or change our therapeutical strategy. The most common clinical situations are discussed in following case histories.
