Abstract
Long term maintenance therapy of Crohn' s disease should be primarily an immunosuppressive therapy. Crohn's disease refractory to classic immunosuppressive drugs (azathioprine) and to biologic therapy (anti TNFa) is therefore a big therapeutic challenge.
The causes of refractority to therapy are varied. It is necessary to carefully analyze possible causes and then modify or change current therapy.
The best way to prevent therapy refractority is good choice of suitable therapy to suitable patient in a suitable time.
