Czech view of new ADA/EASD guidelines for treatment of type 2 DM

Abstract

New ADA/EASD guidelines for treatment of type 2 DM were published in January 2015. Once again, differentiated approach to achievement of target HbA1c is confirmed, taking into account the presence of complications, risk of hypoglycemia, and DM duration.

Metformin remains the medication of first choice. In the second step, it is already possible to choose among six groups of antidiabetics (including the new one - gliflozins), each choice having its advantages and drawbacks.

Many other variants of treatment were thus added even within the third step. In the fourth step, it is possible now to choose among straightforward intensification of insulin therapy, two dosages of premixed insulin, and combinations with incretin analogues.

All algorithms are graphically clear and self-explanatory. Anyone can treat diabetes effectively today, respecting needs and risks specific to individual patients.
