The risks of (onco)gynaecological surgery

Abstract

Despite of advances in chemo- and radiotherapy and introduction of preparations of targeted (biological) treatment, the surgery remains the main therapeutic method in all cases of malignancies of reproductive organs. In some cases (e.g. ovarian cancer), it is the only curative method.

Radical oncogynaecological procedures are the top of gynaecological surgery and are designated for specialized departments. Generally, the more extensive procedure is performed, the more increasing are the possible risks, incidence of complications, and length of hospitalization and recovery.

According to the various criterions, complications can be divided to peroperative, early, and late, to surgical, anaesthesiological, infectious, to local and systemic, etc. The incidence of complications is influenced by a lot of factors like the advanced state of tumour, previous intraabdominal surgeries and radiation, presence of infection, age and associated internal diseases of patient, obesity and others.

The early diagnosis is crucial for the number of complications, along with individualized approach, use of minimally invasive methods, high erudition and experience of operating team and nursing staff, interdisciplinary cooperation, detailed preoperative preparation and qualified postoperative care.
