Monitoring and influencing the determinants of physical and mental functioning of individuals with chronic renal disease and after kidney transplant and its influencing by intervention programs in this project was unique. We have expanded the awareness of the importance of regular physical activity as a non-pharmacological option affecting the health status of individuals on dialysis or after kidney transplant among the professional community of nephrologists, dialysis nurses, physiotherapists, patients and their family members not only at home but also abroad. The results of partial studies confirmed an increased incidence of functional disorders of the musculoskeletal system, reduced physical fitness and lower quality of life of patients after kidney transplant, especially in the first year after the transplant, as compared with a population of healthy individuals.

This status is improving during the first 10 months. However, in comparison with the general population, these individuals still have reduced physical fitness.

The effect of physical fitness on the quality of life of patients after kidney transplant is evident. The combination of physical and nutritional intervention was proven to be the most effective in order to improve physical fitness and subsequently the quality of life.

Diagnostic and interventional procedures applied in the extensive three-year investigation of physical and mental functioning determinants among post-transplant subjects had a very positive response among patients and their physicians. Information about the current state of physical and mental condition and changes over a period of time had positive effect for monitored individuals and supported long-term and regular completion of recommended physical activity programs and consequently helped to increase the volume of physical activities in daily living, which is desirable for improvement of health condition as well as improvement of the level of self-sufficiency and self-care.
