Congenital clubfoot - long term experience with posteroplantar release

Abstract

Introduction: The study presents one of the possible surgical strategies of the therapy of the congenital club foot deformity in infants. Posteroplantar release is used in rigid club feet of Dimegli group 1 and 2.

From the 90's of the last century we prefer this type of operation befor subtalar radical release described by Mc Kay. Material and methods: In the period of 2001-2010 the method of posteroplantar release was used 144 times as a primary surgery.

The youngest patients were 8 months old, the oldest child was during surgery 36 months. In case of the persisting adduction and varus deformity after conservative therapy of the congenital club foot deformity first of all plantar release of the forefoot was performed.

The surgical technique is described in details. Plaster fixation is applied during 3 months after the operation with the replacement of correction plaster casts each 3-4 weeks.

Results: The longes time from the operation is 13 years, the shortest interval after the surgery group of patients is 3 years and 2 months. The followed-up examined clinically with the simultaneous evaluation of radiographs.

Excellent result and good results- anatomical position of the foot was obtained in 66,7%, satisfy result was recorded in 24,9%. Poor result was recorded in 8,4% in children with arthrogryposis multiplex congenita and in children with positive family history.

On the X ray after surgery position of talus and calcaneus an increase of the angle in the lateral projection on average by 13 degrees, in anteroposterior projection by 10 degrees.
