Abstract
The latest class of drugs are glifloziny between whose representative on the Czech market include dapagliflozin, empagliflozin and kanagliflozin [1]. This is a very effective selective and reversible inhibitors of common transporter for sodium and glucose (SGLT2).
SGLT2 It is expressed exclusively in the kidney and is the major transporter, which is reabsorbed in the proximal tubule of glucose from the glomerular the filtrate back into the bloodstream. During its inhibition gliflozinem therefore reduces the reabsorption of glucose by the kidney and increases its excretion in the urine.
Consequently, there is a reduction of plasma glucose concentration both fasting and postprandial. It applies that the higher the blood glucose, the more glucose into the glomerular filtrate gets and the more then glykosurický effect.
This fact implies a more powerful effect on postprandial glycemia. This ultimately results in less fluctuation of blood glucose.
