The self-sufficiency rate in patients on peritoneal dialysis treatment - impact on quality of life

Abstract

Background: Quality of the life (QL) in patients on dialysis treatment in the Czech Republic is significantly reduced in the area of physical health as compared with average values of Czech standards. We have evaluated relationships between levels of independence in activity of daily living (ADL) of patients on peritoneal dialysis (PD) with domains of quality of life.

Group: ESRD patients on PD treatment in the Czech Republic (N=94; male/female = 48/46, average age 56.9+-12.8; average time of PD 29.1+-32.4 months). Methods: WHOQOL-BREF & DIS module (for persons with disabilities) was used to assess QL.

Statistical analysis: descriptive statistics, ANOVA, Wilcoxon and Kruskal-Wallis tests, Person's and Spearman's tests, Cronbach Alpha<0.886, Fischer Eta); p<0.05. Results: Area survey of QL of patients on PD in the Czech Republic showed statistically significant differences in the rating of QL as compared with average values of Czech standards in the domains of WHOQOL- BREF and with values of DIS module.

Rating of the domain "physical health" in PD patients was significantly lower than in the general population (p = 0.00). Significantly higher average rating was found in the domain of "environment" (p = 0.00) and "DIS module" (p = 0.03).

Significant association in the domains "physical health" and "days restricting daily activities due to illness" (p < 0.05) can be found at 1/3 of respondents. There were no significant links between domains "social relations" and "environment" and the number of days with some type of the health problem.

Conclusion: The overall quality of life in patients on PD treatment is closely associated with the degree of self-sufficiency in ADL and with the level of their physical fitness. Independence on the others and self-sufficiency is desirable especially in the elderly and chronically ill patients.

In terms of adherence is essential motivation of the patient to actively participate in influencing their physical and mental health.
