Preexcitation o resting ECG in symptomatic patients (i.e. WPW syndrome) and also in asymptomatic persons (i.e.
WPW pattern) brings increased risk of sudden cardiac death. Estimated lifelong cumulative risk is 3-4 %.
Sports physician deals with preexcitation during preparticipation examination, by chance while performing examination for another condition or when performing stress test as part of risk stratification of known preexcitation. Two case reports present differential diagnostic options and some less frequent findings doctor may have to deal with in aforementioned situations.
