Is it possible to provide integrated care in the Czech Republic? Findigns from long-term action research

Abstract

Background: Health and social care continuity is one of major problems in health and social care for the elderly. Aim of the article is to answer the question whether and under which conditions it is possible to provide integrated health and social care to elderly patients.

Method: Action research, development and evaluation of the geriatric health and social care community based institution Findings: Services are described in a SWOT analysis structure. Conclusion: In the Czech Republic the integrated health and social care provision is not forbidden but there are many obstacles on different levels.

The necessary condition of integrated health and social care provision is the support of municipality which may diminish the impact of some legislative and organizational hurdles.
