Infective endocarditis due to Bartonella quintana: a severe disease and underdiagnosed etiology

Abstract

A case of a 43-year-old man with infective endocarditis with multiple mobile vegetations on mitral and aortic valves, cerebral embolic event and negative blood cultures. Treatment with betalactam antibiotics and gentamicin was ineffective and heart surgery was performed.

Bartonella quintana etiology was detected by PCR of the excised valve and confirmed serologically. The patient survived with severe neurological deficit.
