Abstract

The theme of the whole workshop is framed by the following common questions for all contributions:. Does a cognitive disorder contribute to the patient's health problems, e.g. through non-compliance of medicines or e.g. through malnutrition or dehydration when the patient does not remember eating or drinking?.

Did a cognitive impairment or delirium affect a diagnostic process? E.g. by communicating the symptoms only partially or atypically, due to a cognitive disorder?. Did cognitive impairment affect treatment or convalescence?.

Coordination of care for a person with a cognitive disorder is time consuming - for example, during outpatient care. Likewise e.g. when preparing a discharge from hospitalization.

Can it really work in reality? Can it be said that this is a timely investment that pays off? What are your experiences?. Case-management for a person with dementia - who provides it?.

Can care coordination be considered as cost-effective?
