Abstract

Paratesticular malignant mesothelioma is an extremely rare type of mesothelioma with limited number of published cases. The origin of this cancer is most often associated with exposure to asbestos with long latency between exposure and diagnosis.

We present the case of an 80-years -old patient with hydrocele. Aspiration of this extremly large of the hydrocele was performed a few times prior surgery.

We found large septated, haemorrhagic hydrocele with significantly thickened extranal layers of the testis, smaller testicle of solid consistency and atrophic epididymis. We indicated orchiectomy due to perioperative finding considering the age of the patient.

Histopathological analysis showed malignant mesothelioma of tunica vaginalis. Scrotal resistence was detected in the 6-month follow up.

The resistence was histologically confirmed as recurent malignant mesothelioma after extirpation. Patient is under actively surveillance.
