Rapid effect of TNF blocking therapy in psoriatic dactylitis

Abstract

Psoriatic arthritis is a complex systemic disease involving multiple organs and systems. Dactylitis, diffuse swelling of the fingers of limbs, consists of a combination of arthritis, enthesitis and inflammatory changes in the soft tissues and belongs to the serious and therapeutically difficult manifestations of psoriatic arthritis.

Dactylitis localization on the lower limbs can significantly affect the work ability of the patient. The paper looks into pathogenesis and therapeutic possibilities of dactylitis in the psoriatic disease.

Paper describes case of the patient, in which dactylitis belonged to the most severe manifestation of psoriatic arthritis. Treatment with NSAIDs, methylprednisolone or csDMARDs did not lead to a long-term effect.

Therefore, TNF inhibitor therapy was started with very rapid improvements of dactylitis, onychopathy and enthesitis. The patient could return to full professional and social life.
