The pharmacotherapy review in the polymorbid geriatric patient with focus on the risk of falls and its management

Abstract

Drug-related problems connected with falls are one of the main problems of morbidity and mortality in geriatric patients. They significantly decrease quality of life and increase costs for additional health-care.

The analysis of these drug-related problems by a pharmacist is a tool for seeking the effective pharmacotherapy and minimization of all adverse effects connected with a treatment. The goal of this statement was the revision of pharmacotherapy for geriatric patients in aftercare institution and to point out potential drug-related problems which increase the risk of falls and also other drug-related problems with no relation to the fall.

The other part of the work was searching for possible solutions and eventually performing the interventions which lead to maximization of effect, risk-minimization and to support a patient's adherence to rational pharmacotherapy.
