Abstract
The document is a follow-up to the "Recommendations for implementing case management in dementia care (toolkit)". It presents a set of indicators and a proposal for their evaluation.
In developing these indicators and considering their optimal and standard fulfillment, we relied on experience from practice, previous projects, both with indicators of long-term care quality (1) and with the certification system of the Czech Alzheimer's Society, "Vážka" (2), based on the P-PA-IA (3) as well as FACT Fidelity Scale (4), in particular from the proposals of the participants of the workshops associated with the project.
