Abstract
This monograph presents outcomes of the research project which focused on case management in the care of persons with dementia. It presents some of its results and provides an overview of the dementia syndrome management, cost-analysis, organisation of services, barriers and facilitators in its provision.
It presents also some good practice examples - care organization in Westminster, Bergent and The Netherlands. It includes proposal of case management standards and also a toolkit for the implementation.
