Sleep breathing disorders : surgical treatment

Abstract

Sleep breathing disorders are among the most common sleep disorders, and their incidence and costs associated with diagnosis and treatment are rising worldwide every year. Diagnosis and treatment of these disorders is complex and involves a number of disciplines (ENT, dental surgery, neurology, pneumology, internal medicine, paediatrics), which often otherwise only cooperate only marginally.

This unique monograph dedicated to the surgical treatment of sleep disorders, particularly obstructive sleep apnea (OSA) and ronchopathy, is the first ever publication on the topic in the Czech Republic. The monograph consists of 30 chapters.

The general part is devoted to history, anatomy, pathophysiology and detailed classification of sleep respiratory disorders, as well as symptomatology and diagnostics, using the latest knowledge in the field. For example, the modern examination method of induced sleep endoscopy (DISE) is described in great detail, including its correct performance and evaluation.

Although the book is primarily focused on surgery, it also deals with non-surgical methods of treatment of OSA, especially with treatment of overpressure in the upper airways and mandibular protractors. A special part of the monograph is fully devoted to surgical methods to remove OSA and snoring.

There are described in detail the procedures in the area of velopharyngeal and retrolingual straits, which make up the largest part of sleep surgery, both basic procedures such as ulcer assisted uvuloplasty or uvulopalatopharyngoplasty and their variations, as well as state-of-the-art methods such as robotic tongue root surgery or hypoglossal nerve stimulation. Other chapters are devoted to less frequent procedures performed for breathing disorders in sleep, such as surgery of the nasal cavities, larynx or jaw and bariatric surgery.

In all these operations, emphasis is placed on a detailed description of the method, including correct indication, selection of anesthesia, description of possible complications as well as anticipated effectiveness in the elimination of sleep disorder. Thanks to its overall concept, the monograph forms a broad basis for the study of treatment of breathing disorders in sleep not only for specialists - somnologists, but also for ENT doctors or other specialists
