Abstract

The article describes a case report of a 78-year-old polymorbid patient who has beencontinuously hospitalized for the last 11 months in the Department of Internal Me-dicine of the University Hospital Motol and in two aftercare centres because of re-current cholangitis and loss of self-sufficiency. On admission to our department, the patient was treated for type 2 diabetes with three injections of regular insulinbefore main meals.

This treatment led to hypoglycaemia alternating with hypergly-caemia. The situation improved significantly after switching to basal insulin deglu-dec.

However, during the second stay at the aftercare centre, the patient returned tothe original treatment regimen, which resulted in diabetes destabilisation and severehypoglycaemia. After the patient's transfer back to our department, we resumed theproven insulin degludec therapy, which, in combination with the education of thepatient's daughter regarding diabetes, finally allowed the patient to be released tohome care.
