Are the Role of Surgery and its Complications Sufficiently Focused in the Era of Perioperative Systemic Treatments?

In summary, there is some surgery-related guidance that should be respected in our efforts to optimize the management of patients with localized MIBC:

1) As long as systemic treatments do not have reliable curative potential, radical surgery remains an integral and crucial part of disease management.

2) Perioperative complications should be reported after prospective data collection based on EAU guidelines.

3) Efforts to reduce the morbidity of radical surgery should focus on prevention and solution of individual clinical problems and not just on implementation of new technologies.

4) Data on surgery, its extent, quality criteria, complications, and outcomes, as well as the expertise of the surgeon and the hospital team, should be comprehensively registered, evaluated, and reported in all trials dealing with new modalities for systemic perioperative treatment.
