Imaging modalities have been widely used in diagnostics of idiopathic inflammatory myopathies and their importance continues to grow. MRI of the thigh muscles became the most often used imaging method.
This modality provide detalied imaging of soft tissues and ongoing pathological processes, it can be helpful in guiding muscle biopsy and improving its diagnostic yield. The most frequently assessed morphological aspects are muslce oedema (areas of increased vascular permeability), muscle atrophy and fatty replacement following long-lasting muscle inflammation or denervation.
There is still no universally accepted and validated scoring protocol for the evaluation and grading of muscle MRI pathology in patients with IIM.
