Immunotherapy of lung cancer and basic principle of immune system acting

Abstract

Mechanisms of cellular immunity play the key role in anticancer immunity. Antigen presenting cells and cytotoxic T lymphocytes are the main actors.

Cytotoxic T lymphocytes are responsible for cancer cell killing. Metastatic lung cancer is an illness with very poor prognosis. 5years survival does not exceed 5%.

Checkpoint inhibitors in monotherapy or its combination with chemotherapy is the 1.line standard treatment for non-small cell and small cell lung cancer stage IIIB and higher. Only in cancers with sensitive drover mutation with accessible targeted therapy, this treatment modality is preferred.

If it is impossible to deliver immunotherapy in 1.line, it can be delivered in second line. First line treatment is preferred, because only in this way is possible to obtain the maximal survival benefit, provided by the therapy.

Checkpoint inhibitor treatment should be administered in all lung cancer patients, if it is possible.
