Patient's values, preferences and concerns and also personal relationships, troubles and symptoms represent the main topics discussed during the initial consult with palliative care team in hospital setting. The consult must be documented in written report.
The report may serve as a communication tool to facilitate communication between the patient / parents and health care providers, patient's family and also with palliative care providers outside the hospital. The article offers an insight into the way of reporting palliative care consult in two university hospitals in Prague, Czech Republic, in both adult and pediatric palliative care teams.
Authors share their ideas about leading the palliative care consult and the aspects of preparing the written report and they offer several recommendations for clinical practice.
