Invasive aspergillosis (IA) is a significant cause of morbidity in immunocompromised patients. A. calidoustus has been introduced relatively recently as a member of Aspergillus section Usti.

Due to its intrinsic in vitro resistance to azoles, it is often found in breakthrough infections in patients on prophylactic antifungal therapy. Our report concerns a 30-year-old male with a medical history of primary cellular immunodeficiency, atopic asthma, gastroesphageal reflux disease and polyvalent allergy with repeated endoscopic surgeries of the nasal cavities in the last two years, which were complicated by an invasive aspergillosis caused by A. fumigatus and A. flavus respectively.

The infections were treated twice with voriconazole.
