Efficacy of unithiol (2,3-dimercaptopropanesulfonate) and acetylcysteine in a patient with arthroplastic cobalt toxicity

Previously we reported a 56-year-old man with severe cobalt poisoning from a damaged metal-on-ceramic hip prosthesis. He was treated with a total of 10 g of oral unithiol (2,3-dimercaptopropanesulfonate; DMPS) over 15 days beginning 2.5 weeks after his prosthesis was removed.

His urinary cobalt concentration increased from 137 µg/L to 305 µg/L and his serum cobalt concentration decreased from 506 µg/L to 130 µg/L over four weeks, during which time his symptoms almost completely resolved. Based on this experience we treated two more patients who are reported herein.
