Asymptomatic primary hyperparathyroidism - to operate or not operate?

Abstract

Primary hyperparathyroidism is a relatively common endocrine disease with typical clinical symptoms and laboratory characteristics. Surgery is a therapy of choice.

Nowadays however, asymptomatic forms of the disease with few or missing clinical signs prevail, with uncertainty as to the potential progression during long-term follow-up. Controversy regarding optimal therapy - conservative or surgical persists.

As prediction of the disease course is difficult, surgical treatment is probably optimal at least for all patients with successful localization of enlarged parathyroid gland.
